Medical Abortion by Trophoblast-Targeted Nanopartciles Loaded with methotrexate

英文摘要Medical abortion (MA) is an increasingly preferred option for pregnancy termination in early gestation in many countries. Several drug regimens are used with varying efficacy including mifepristone (MIFE), misoprostol (MISO) and methotrexate (MTX), but they all have some side effects. Here we report a technology potentially overcoming these shortcomings and enhancing their efficacy through a trophoblast targeted nanoparticle (pl-CSA-MNPs) made by using a placenta chondroitin sulfate A ( pl-CSA) binding peptide, which is specifically binding to the pl-CSA expressed only in the trophoblast cells of placenta, to coat the MTX loaded lipid-polymer nanoparticles (MNPs). Free MTX, MNPs and MNPs coated by a peptide with random sequence of the same amino acids as the pl-CSA binding peptide (SCR-MNPs) were used as control groups. The produced nanoparticles was sphere shape with diameters around 110nm (pl-CSA-MNPs), 100nm (SCR-MNPs) and 70nm (MNPs). Meantime, the biotinylated pl-CSA binding peptide (Bio-pl-CSA) could efficiently bind to mouse and human placental trophoblast cells. Then high-frequency ultrasound (HFUS) was used to monitor the fetuses’ growth in different gorups throughout gestation; meantime Doppler measurements of fetal heart rate (HR) and umbilical blood flow were record to evaluate fetal blood supply and wellbeing. The results showed that, by intravenous injection (i.v) of pl-CSA-MNPs, the conceptus growth was retarded which exhibited a small embryonic cavity. Then, the echodensity of the embryonic fluid was increased and blood flow velocity through the umbilical artery (UA) and vein (UV) was decreased within two days. The corresponding histologic changes are as following fibrinoid changes were surrounded by material granulocytes and lacunae filled with maternal blood were observed at the placenta-maternal interface. In later stages (day 12-14), fetuses showned bradycardia and pericardial edema, ceased development and about 80% fetuses death that was 3 times higher than other groups. The Tunel assay indicated that the placental trophoblast cells apoptosis was increased, while the toxicity of MTX to the liver and kidney was significantly reducing in the pl-CSA-MNPs compare to other groups. Collectively, these results highlight pl-CSA-targeted NPs as a promising and effective alternate regimen for early MA.
