This paper describes development of a web-based visualization tool and data acquisition for improving the community healthcare. The system is applied to obtain patient behaviors of medical treatment in a community. The data are based on survey data collected by social scientists. Using our system, users obtain the visualization images efficiently without specialized knowledge of visualization and network technology, usefulness of the tool and its possibility with intersystem coordination. Relations of the visualization tool, regional information and survey data are discussed.
