中文摘要Objective:As a common problem in many workplaces, job burnout is particularly prominent to nurses who are major group of health care services, causing negative effect on their physical and mental health as well as organization development. Current researches on personality traits, coping style and job burnout suggested that Personality has an effect on job burnout through coping style. However, it is difficult to intervene and improve burnout from personality due to its relatively stable and lasting psychological behavior tendency. Meanwhile, Psychological flexibility is a manipulable personal trait. It can be enhanced by acceptance and commitment therapy. People with Higher psychological flexibility can choose coping strategies flexibly according to specific situations, which can reduce the occurrence of burnout and maintain their physical and mental health. Therefore, this study takes psychological flexibility as the starting point and coping style as the clue, and finds a new solution for the intervention of job burnout by exploring the relationship among psychological flexibility and coping style and job burnout of nurses in tertiary hospital and secondary hospitals respectively.Methods:The study was composed of two studies.Study 1: To explore the relationship among psychological flexibility, coping style and job burnout of nurses in tertiary hospital. Totally 694 nurses from one district level grade A tertiary general hospital in Yunnan were assessed using Acceptance and Action Questionnaire 2nd edition (AAQ-Ⅱ), Simplified Coping Style Questionnaire (SCSQ) and Nursing Burnout Scale (NBS). SPSS23.0 was used to verify the research hypothesis of correlation and predictive effect among psychological flexibility, coping style and job burnout. Mplus7.0 was used to test the mediating effect of coping style between psychological flexibility and job burnout. Study 2: To explore the relationship among psychological flexibility, coping style and job burnout of nurses in secondary hospitals. Totally 1182 nurses from one district level nine grade A secondary general hospitals in Yunnan were assessed using the same questionnaires and statistical methods as those in study 1. Finally, the nurses’ psychological flexibility, coping style and the total scores of job burnout were compared and analyzed between the tertiary and secondary general hospitals, and Mplus7.0 was used to compare the mediating effect of coping style between the two levels hospital nurses.Results:Study 1: In tertiary hospital nurses, psychological flexibility was significantly positively correlated with positive coping, and negatively correlated with negative coping and the total scores of job burnout and its three dimensions including emotional exhaustion, depersonalization, as well as reduced personal accomplishment. Psychological flexibility of nurses in tertiary hospital had a significant positive predictive effect on positive coping, and a significant negative predictive effect on negative coping and the total scores of job burnout and its three dimensions. Psychological flexibility and positive coping had significant negative predictive effects on the total scores of job burnout and its three dimensions. While negative coping had a significant positive predictive effect on the total scores of job burnout and its three dimensions. Coping style partially mediated psychological flexibility and job burnout in tertiary hospital nurses. Compared with positive coping, negative coping played a greater mediating role.Study 2: In secondary hospital nurses, psychological flexibility was significantly positively correlated with positive coping, and negatively correlated with negative coping and the total scores of job burnout and its three dimensions. Psychological flexibility of nurses in secondary hospitals had a significant positive predictive effect on positive coping, and a significant negative predictive effect on negative coping and the total scores of job burnout and its three dimensions. Psychological flexibility and positive coping had significant negative predictive effects on the total scores of job burnout and its three dimensions. While negative coping had a significant positive predictive effect on the total scores of job burnout and its three dimensions. Coping style partially mediated psychological flexibility and job burnout in secondary hospital nurses. Compared with positive coping, negative coping played a greater mediating role. Comparing two groups of nurses, the tertiary hospital nurses’ psychological flexibility, coping style and the total scores of job burnout were higher than the secondary hospital nurses. Comparing three dimensions of job burnout, depersonalization of nurses in tertiary hospital was significantly higher than that in secondary hospitals. There were no statistical difference in the mediating effects of coping style between the tertiary hospital nurses and secondary ones.Conclusion:First, coping style of nurses in the tertiary hospital has mediating effect on the relationship between psychological flexibility and job burnout. Negative coping plays a more mediating role than positive coping.Second, coping style of nurses in the secondary hospitals has mediating effect on the relationship between psychological flexibility and job burnout. Negative coping plays a more mediating role than positive coping.Third, the mediating effect of coping style between psychological flexibility and job burnout is the same between the two groups of nurses. Acceptance and commitment therapy can be used to improve nurses' psychological flexibility and reduce negative coping style, so as to prevent and reduce their job burnout.
英文摘要目的:职业倦怠是诸多工作场所的一个普遍问题。护士是医疗保健服务的主要群体,其职业倦怠现象尤为突出,给护士的身心健康和组织发展带来不良影响。目前关于人格特质、应对方式和职业倦怠的研究认为,人格通过应对方式影响职业倦怠的发生。但由于人格具有相对稳定和持久的心理行为倾向,难以从人格层面对职业倦怠给予干预和改善。心理灵活性是一个可操纵的个人特征,可以通过接纳承诺疗法得以提升。高心理灵活性的人会根据具体的情境灵活地选择应对策略,使倦怠的发生减少,保持其身心健康。因此,本研究以心理灵活性为起点,以应对方式为线索,分别研究了三级医院和二级医院护士群体的心理灵活性、应对方式和职业倦怠的关系,旨在为职业倦怠的干预提供新的解决思路。方法:本研究由两个研究组成。研究一:三级医院护士的心理灵活性、应对方式与职业倦怠的关系检验。整群选取云南省某地市级一所三级甲等综合性医院临床护士694人,分别测量心理灵活性、应对方式、职业倦怠的水平,用SPSS23.0软件对心理灵活性、应对方式、职业倦怠间的相关关系和预测作用的研究假设进行验证,用Mplus7.0软件进行应对方式在心理灵活性与职业倦怠间的中介效应检验。研究二:二级医院护士的心理灵活性、应对方式与职业倦怠的关系检验。整群选取云南省某市9所二级甲等综合性医院临床护士1182人,施测问卷和统计方法与研究一相同。最后对三级医院和二级医院护士的心理灵活性、应对方式及职业倦怠程度进行对比分析,并用Mplus7.0软件对两级医院护士应对方式的中介效应进行群组比较。结果:研究一,三级医院护士的心理灵活性与积极应对显著正相关,与消极应对、职业倦怠总分及其3个维度:情感耗竭、人格解体、成就感降低显著负相关。三级医院护士的心理灵活性对积极应对有显著正向预测作用,对消极应对、职业倦怠总分及其3个维度有显著负向预测作用;心理灵活性和积极应对对职业倦怠总分及其3个维度有显著负向预测作用,消极应对对职业倦怠总分及其3个维度有显著正向预测作用。三级医院护士的应对方式在心理灵活性与职业倦怠间有部分中介效应。与积极应对相比,消极应对起着更大的中介作用。研究二,二级医院护士的心理灵活性与积极应对显著正相关,与消极应对、职业倦怠总分及其3个维度显著负相关。二级医院护士的心理灵活性对积极应对有显著的正向预测作用,对消极应对、职业倦怠总分及其3个维度有显著的负向预测作用;心理灵活性和积极应对对职业倦怠总分及其3个维度有显著负向预测作用,消极应对对职业倦怠总分及其3个维度有显著正向预测作用。二级医院护士的应对方式在心理灵活性与职业倦怠间有部分中介效应。与积极应对相比,消极应对起着更大的中介作用。比较两个护士群体,三级医院护士的心理灵活性、应对方式的2个维度:积极应对和消极应对以及职业倦怠总分均显著高于二级医院护士。比较职业倦怠的3个维度,三级医院护士的人格解体显著高于二级医院护士。三级医院护士与二级医院护士应对方式的中介效应比较无统计学差异。结论:一、三级医院护士群体的应对方式在心理灵活性与职业倦怠间有中介效应。消极应对较积极应对有更大的中介作用。二、二级医院护士群体的应对方式在心理灵活性与职业倦怠间有中介效应。消极应对较积极应对有更大的中介作用。三、两个护士群体的应对方式在心理灵活性与职业倦怠间的中介效应程度相等。可采用接纳承诺疗法来提高护士的心理灵活性,以降低消极的应对方式,从而预防和减少职业倦怠的发生。
