自我抽离对失眠障碍共病焦虑和抑郁的疗效

中文摘要Objective: To investigate the effect of self-distance training on the improvement of negative mood and sleep quality in patients with insomnia disorder comorbid anxiety and depression, and to provide evidence for the further promotion and application of self-distance training as an auxiliary therapy in clinic. Methods: A total of 227 patients diagnosed with insomnia disorder comorbid anxiety and depression from January 2020 to May 2021 in Yunnan Psychiatric Hospital were selected. 56 patients who did not meet the inclusion criteria were excluded. The remaining 171 patients were divided into 4 groups according to patients' wishes, namely drug treatment group (n=48), which received drug intervention during the study self-distance group (n=43) received self-distance training intervention; drug combined self-distance group (n=42) received combined drug and self-distance training intervention; The control group (n=38) received no intervention except safety and health education. After 12 weeks of continuous implementation, the anxiety of the patients was evaluated by Hamilton Anxiety Scale (HAMA)and Self-rating Anxiety Scale (SAS) at baseline, 2 weeks after intervention, 4 weeks after intervention, 8 weeks after intervention and 12 weeks after intervention. The depression status of the patients was assessed by HAMD and SDS. The sleep status of the patients was investigated by using the Pittsburgh Sleep Quality Scale (PSQI). Finally, data were collected and analyzed with SPSS 26.0. Results: After removing 8 cases of shedding subjects, 163 cases of valid data were obtained. The results of repeated measure variance analysis showed that the grouping effect, time effect and grouping X time effect of HAMA, HAMD, SAS, SDS and PSQI scores were statistically significant (ps<0.001). Further analysis of the separate effects showed that there were no significant differences in HAMA, HAMD, SAS, SDS and PSQI scores at baseline in the drug treatment group, self-distance group, drug+self-distance group and control group (ps>0.05). The self-distance group had lower scores than the control group at 4, 8 and 12 weeks after intervention勿s<0.05). At 8 and 12 weeks after intervention, the scores ofthe drug+self-distance group were lower than those of the drug+self-distance group (ps<0.05). At 4, 8, and 12 weeks after intervention, the scores of the drug treatment group, self-distance group, and drug+self-distance group were all lower than baseline (ps<0.05). Conclusion: Self-distance training can improve sleep quality and relieve negative emotions such as anxiety and depression in patients with insomnia disorder. The effect of this training combined with drug therapy is better than that of drug therapy alone. Self-distance training plays an adjunctive role in the treatment of insomnia comorbid anxiety and depression.
英文摘要目的:考察自我抽离训练对失眠障碍共病焦虑和抑郁患者的负性情绪及睡眠质量的改善效果,为进一步把自我抽离训练作为辅助治疗的方法推广运用于临床提供依据。 方法:选取云南省精神病医院2020年1月至2021年5月诊断为失眠障碍共病焦虑和抑郁的患者227例,剔除不符合入组标准56例,剩余171例,根据患方意愿分为4组:即药物治疗组(n=48),在研究过程中予药物干预;自我抽离组(n=43),给予自我抽离训练干预;药物+自我抽离组(n=42),联合使用药物及自我抽离训练干预;对照组(n=38),除安全健康宣教外不给任何干预。持续实施12周,分别在基线、干预开始后2周、干预开始后4周、干预开始后8周、干预开始后12周采用汉密顿焦虑量表(HAMA )、焦虑自评量表(SAS )对患者的焦虑情绪进行评估;采用汉密顿抑郁量表(HAMD )、抑郁自评量表(SDS)评估患者的抑郁状态;运用匹茨堡睡眠质量表(PSQI)对患者的睡眠情况进行考察。最后收集数据,用SPSS 26.0进行数据分析。 结果:除去脱落被试8例,最后获得有效数据163例,对其进行重复测量方差分析结果显示,HAMA, HAMD, SAS, SDS, PSQI得分的分组效应、时间效应及分组X时间效应均有统计学意义(ps < 0.001)。进一步分析其单独效应显示,药物治疗组、自我抽离组、药物+自我抽离组及对照组在基线的HAMA, HAMD, SAS, SDS, PSQI评分均无显著性差异(ps > 0.05 );自我抽离组在干预开始后4, 8, 12周的评分均低于对照组(ps < 0.05 );药物+自我抽离组在干预开始后8, 12周的评分均低于药物治疗组和自我抽离组(ps < 0.05 );药物治疗组、自我抽离组、药物+自我抽离组在干预开始后4, 8, 12周的评分均低于基线(ps<0.05)。 结论:自我抽离训练可以提高失眠障碍共病焦虑和抑郁患者的睡眠质量,减轻其焦虑、抑郁等负性情绪;该训练联合药物治疗的效果优于单纯药物治疗;自我抽离训练对失眠障碍共病焦虑和抑郁有辅助治疗的作用。
